This is the application form for the Kuantum Education Scholarship Exam. To take part in the exam, the following information must be filled in completely: Full Name, Mobile Phone, Your Current School, Your Current Grade Level, Parent's Full Name, Parent's Mobile Phone, Parent's Relationship, Select a Session, How did you hear about us? and Select a City.

Grade levels eligible to apply: 5th Grade, 6th Grade, 7th Grade, 8th Grade, 9th Grade, 10th Grade, 11th Grade Science Track, and 11th Grade Equal Weight Track.

Exam sessions: Saturday, January 10 at 10:00 Saturday, January 10 at 14:00 Sunday, January 11 at 10:00 Sunday, January 11 at 14:00

After filling in the form, the application is completed using the "Submit My Exam Application" button.
